Estimated population per doctor (1:X) at government hospitals by state—lower X means more doctors per person. WHO planning commonly cites about 1:1000 (one doctor per 1,000 people) as a reference density. Malaysia has discussed higher targets (e.g. ~2.5 per 1,000 by 2025, ~1:400; Ismail, 2023).
